Patient Forms, Map and Directions to Our Crystal Lake Office

Your first visit to Osmanski Dental establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. To understand what to expect for your first visit to our practice, please read through this page. You can save some time at your first visit by printing out and completing the patient forms in advance of your appointment.

Please assist us by providing the following information at your first visit:

  • List of medications you are presently taking, especially if it is heart medication, aspirin or an anticoagulant therapy
  • Please alert us if you have any type of medical condition (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.)
  • Please let us know if you require medication prior to dental cleanings (antibiotic, for pre-med) -  We can supply you with a prescription for this prior to your first visit
  • If you have dental insurance, bring the necessary forms or card

A parent or guardian must accompany all patients under 18 yrs of age at the first visit.


What To Expect At My First Visit 


My team and I appreciate your selection of our office to serve your dental health needs. Our primary concern is to make you feel comfortable in our office by providing a relaxed enviroment and taking extra time to understand your wants and needs.  

During your first visit, you will receive a through examination  and diagnosis.  This will include x-rays of your teeth, a health history, and an oral examination.  This examination will include charting of the teeth/gums, checking for decay, fractures and cracks, evaluating your bite and current restorations and performing an oral cancer screening.  These diagnostic tools enable us to develop a personalized treatment plan to address your current and long-range goals.  

In most cases, during your first visit, we will perform your oral hygiene treatment, but a complex medical history or treatment plan may require a second appointment.  
In order to allow enough time to perform the comprehensive exam and the hygiene visit, please allow 2 hours for your first visit with us. Should you have any questions about the forms to bring with you to your first visit, please call our office and any member of our team will be happy to assist you.
